Location : Hybrid  in El Paso, TX

Duration :   11 week contract

Shift :   M-F 8-5 MST

Start Date :   Monday April 7th, 2025

Requirements :

  • LVN, RN, or bachelor's degree in Social Work
  • 2 Years of Experience in a Community Health setting
  • Ability to travel in El Paso, TX area to visit members in their homes or other settings such as community centers and hospitals
  • Bilingual in English and Spanish

Essential Job Functions :

  • Scheduling and conducting face to face and / or telephonic member assessments
  • Coordinating member’s care (needs identified during assessments)
  • Completing all required paperwork within state specific turnaround times
  • Collaborating with member and their LAR to develop an individualized plan of care
  • Completing comprehensive daily / weekly status and handoff reports
