Shift: 8:00am- 4:00pm Monday- Friday
Pay Range: $35-$45
Must haves:
- 2 years of experience as a Case Manager in an acute care or rehab setting
- RN license or LSW or LCSW or CSW in New Jersey
- Experience with utilization review, discharge planning, performing documentation for patients
- Excellent communication and knowledge of basic computer skills

Job Description:
Insight Global is seeking an experienced Case Manager to join the Kessler Rehabilitation team. This role includes discharge planning, documentation, utilization review, and patient chart review. You’ll ensure smooth patient transitions from the hospital and maintain accurate records. Additionally, you’ll provide education to patients and families and participate in quality improvement initiatives. You’ll work alongside two physicians and manage a caseload of up to 18 patients, who typically stay for up to 2 weeks. They are looking for someone with strong organizational skills and proficiency in computer skills who will be an asset to the team.
Salary : $35 - $45
