We do this through specialized day programs for individuals living with memory and cognitive impairments, support for care partners and families, and education and engagement opportunities for the wider community. Position Description
The Program Director (PD) at IMCC is part of a team providing direct support to families and participants. The PD will be responsible for developing and implementing participant care plans, overseeing the social aspect of care for participants, maintaining quality and compliance with Center policies and regulations, and linking care provisions with family members. The PD will maintain a professional relationship with service users and act as a guide and advocate. The PD is responsible for creating an environment that is uplifting, stimulating and offers participants the opportunity to be successful. This position also functions as a role model for staff during interactions with participants and other staff. This position reports directly to the Director of Operations.
- Coordinates and assists with behavioral, recreational and social services including orientation, communication, responding to challenging behaviors, socialization and activities, supplemental advice and support.
- Ensures that treatments are administered by Nurse and documented in accordance with policy. Develops and implements specific protocols for addressing other medical, nursing and behavioral needs.
- Provide individual and family care consultation for family caregivers that include reassurance, referral, information, education and active listening.
- Develop and maintain knowledge base of community resources and care providers for those with dementia.
- Maintain on-going awareness of the individual needs of participants, noting changes in functional ability, and the possible need to move to another level of care.
- Maintain communication with family caregivers concerning the well-being of participants.
- Serve as primary for inquiry phone calls and center tours.
- Meet with families to complete signing enrollment paperwork with responsible party (i.e. contract and admission paperwork).
- Work with Director of Recreation in developing new and innovative activity programs based on the interests and remaining abilities of participants, via monthly calendar and events.
- Assures the centers adherence to Federal and State regulations and certificationMonitor the process of participants obtaining annual physicals by the required dates with Director of Nursing (and manage in their absence)In conjunction with Director of Nursing, develops protocols for supervising participant administration of medication.
- Ensure that participant files are complete on admission and audit on a quarterly basis to ensure ongoing compliance.
Education
- Masters Degree in Social Work or Licensed Social Worker
Qualifications
- 2-3 years long-term care experience, gerontological experience including dementia population required; must have effective communication skills; demonstrated supervisor skills; ability to read charts.
- Must be able to read, write, and speak the English language in an understandable manner.
- Finally an understanding of the Commonwealth of Virginia's Department of Social Services Regulations on Adult Day Health Care Services is preferred.
