Nevada Kidney Disease and Hypertension Center (NKDHC) is the largest provider of dialysis products and services. With over 1,800 kidney dialysis clinics, we provide superior care to individuals with renal (kidney) disease. We are considered experts in the treatment of Chronic Kidney Disease (CKD), End Stage Renal Disease (ESRD), Acute Renal Failure (ARF), Kidney Transplantation and creating and maintaining accesses for dialysis therapies. Our company is a professional medical association consisting of physicians and other vital support staff.
Embedded Resource Specialist
In furtherance of the Treatment Program, the Embedded Resource shall monitor and coordinate patients' treatment plans, reinforce education about their condition, and coordinate care with healthcare providers and partners. The responsibilities and activities include the following:
Responsibilities and Actions:
- Actively engage a panel of CKD and ESRD patients. Follow-up with patients to ensure their needs are met and schedule future meetings.
- Build relationships with patients, working alongside the interdisciplinary team, including nephrologists, renal nurses, advanced practice providers, pharmacists, social workers, and dietitians.
- Partner with Optum and Provider resources to manage all aspects of care related to resources, appointments, care transitions, and care gaps.
- Work collaboratively with Optum resources to facilitate outpatient services.
- Provides regular in-person and telephonic interventions to patients with a focus on care plan adherence and barrier resolution.
- Communicate with patients in-person, by video or telephone and escalate to the care team when necessary.
- Link patients to community resources.
- Track census on inpatient admissions, coordinate care, ensure the proper discharge, and timely follow-up as appropriate.
- Monitor inpatient admissions and follow-up as necessary with care team members and outside resources to confirm all relevant patient information.
- Assist with the coordination of care across the kidney care continuum, such as scheduling appointments with specialists and dialysis centers, coordinating referrals, and sharing or transferring information with the patient’s care team.
- Assist with enrollment in services and community resources.
- Assist with completing applications for resources, paperwork for provider visits, etc.
- Assist with finding and scheduling transportation.
EDUCATION:
- High School Diploma required; Medical Assistant (MA) Certificate desirable; and Licensed Practical Nurse (LPN) strongly preferred
EXPERIENCE AND REQUIRED SKILLS:
- 2 – 4 years’ customer service-related experience in a medical office role
- Excellent verbal and written communication skills
- Organization, planning, and problem-solving skills
- Experience with software applications typically used in a medical office. This may include but is not limited to electronic medical office systems such as: electronic medical records, scheduling programs, switchboard, credit card machines, standard office equipment and any new technology that arises.
