MAJOR FUNCTION: The Ambulatory Care Clinical Pharmacist provides comprehensive medication management services to patients in the primary care setting. Integrating pharmacist into primary care can prevent avoidable spending by increasing patient adherence, optimizing prescription regimens, and preventing medication related complications. Many medication problems are related to clinician-influenced gaps in care, including inappropriate or ineffective prescribing, lack of care coordination, and inconsistent monitoring. The Ambulatory Care Clinical Pharmacist will bridge these gaps through multidisciplinary collaboration with primary care physicians and the primary care team. QUALIFICATIONS: Education Experience: College or School of Pharmacy Accredited by American Council on Pharmaceutical Education required. ASHP PGY2 Residency Preferred. ASHP PGY1 Residency Preferred. Without ASHP PGY2 residency 5 years of clinical experience. Certification/ Licensure: Active New Jersey Registered Pharmacist license required. Active Basic Life Support (BLS) certification is required.
