MAJOR FUNCTION: The Population Health Population Care Coordinator (PCC) will work on a multidisciplinary health care team in a primary care setting. The PCC will focus on coaching and coordination of care for high risk and at risk patient populations. The PCC will work alongside providers and office staff engaged with the Population Health department, focusing on identifying the needs of these members while assisting the practice in developing processes for managing these populations. The PCC role is a key role in the managing the needs of patients with complex needs and chronic conditions in order to improve the overall health of local populations. QUALIFICATIONS: Education Experience: Associates degree required. Bachelor's degree preferred. Experience in acute inpatient, rehabilitation, sub-acute, skilled facility, homecare or managed health plan. Experience in a care management role preferred. 5 to 8 years of direct patient care experience required. Familiarity with community resources and social service resources to assist patients with social needs. Certification/Licensure: Requires a New Jersey Registered Nurse License. Certified Case Management (CCM) certification preferred. ?>
