  • SUMMARY: The Service Coordinator is primarily responsible for the facilitation of child and adult - centered services and supports. This position requires the coordination of service delivery identification of resources in response to family/client needs. The Service Coordinator will provide Service Coordination services in various areas including; Service Coordination (EI), Part C Services, Children's Extensive Support (CES), Home Community Based Programs for the Developmentally Disabled (HCBS-DD), Supported Living Services, both Waiver and State (SLS), Service Coordination Only, etc. In addition, this individual will provide direct monitoring of contracted professionals. The Service Coordinator is responsible for resource allocations in each of the assigned programs and to work toward the goals of the individuals in service and the agency in a person-centered environment.
  • ESSENTIAL DUTIES and MINIMUM QUALIFICATIONS:
  1. Responsible for the facilitation of receiving services and supports for family/persons applying for service, as well as termination and/or transfer of services and supports within established timelines.
  2. Coordinate initial and all other Individual Family Service Plan / Individual Plan meetings to develop a plan based upon client needs which includes the implementation and monitoring of services and supports. Ensure schedules of service changes are completed, submitted as necessary, and expenditures are within guidelines established by the State of Colorado.
  3. Perform billing functions for children in service in accordance with established agency, state and federal rules and regulations and within established timelines.
  4. Maintain regular contact with children and adults in service to assess satisfaction with services, and advocate in collaboration with and/or on behalf of the client to ensure the individuals' rights to access appropriate services and opportunities. The Service Coordinator will act as a knowledgeable liaison between consumers, families, guardians, program staff, other agencies, providers, and the agency.
  5. Ensures appropriate maintenance and completeness of the individuals' master files including the periodic purging and/or destruction of records in accordance with agency policies and procedures.
  6. Pass the Medication Administration test with no more than two attempts and administer medication to clients according to physician orders and using appropriate administration techniques as outlined in the Medication Administration course, if needed.
  7. Must be able to perform Lifting and Transferring techniques as trained and pass performance testing. The employee may occasionally lift and/or push and pull up to 50 lbs. Lifting may require floor to waist, waist to shoulder, or shoulder to overhead movement.
  8. Driving of agency vehicles as necessary for transportation of clients and agency business as assigned.
  9. Must, with or without reasonable accommodation:
  • Be able to regularly sit, walk, stand, climb up and down stairs, physically restrain clients pursuant to training (when necessary) and sit at a computer for long periods of time and repeat the same hand, arm, or finger motion many times (typing, data entry).
  • Communicate through speech with individuals in service and public in both quiet and noisy environments.
  • Operate a personal computer and related software.
  • Tolerate various levels of stress and meet deadlines with severe time constraints.
  • Demonstrate specific vision abilities which include close, distance, and depth perception and are able to read handwritten or typed material, and able to adjust focus.
  • Work and concentrate in both noisy and quiet environments.
  • Squat, stoop, kneel, and reach above the head. Forward motion will occasionally be required.
  • Work inside the building a majority of the time.
  • Essential Duties include the Physical Demands listed below.

ADDITIONAL DUTIES AND RESPONSIBILITIES:

  1. Obtain program accreditation through Early Intervention and Colorado Department of Health Care Policy and Financing and Service Coordination and/or other regulatory agencies and maintain program approval with few, if any, deficiencies for all assigned areas of responsibility.
  2. Plan and participate in agency events.
  3. Responsible for prior-authorization of Medicaid waiver services within established timelines.
  4. May be assigned by the Supervisor or the Chief Executive Officer or their designee, to investigate allegations of Abuse/Neglect/Mistreatment/or Exploitation of persons receiving services.
  5. Recruit, set-up, assist, monitor, and maintain contract providers and resources for the purpose of connecting families and individuals in service with the community. Ensure service providers directly receive the appropriate information/training regarding the needs of children and adults in service.
  6. Maintain all programs' databases, inputting vital information, and compiling reports as required by agency, state, and federal rules and regulations.
  7. Provide information and events that promote the Children and Adult Programs community wide, and facilitate training opportunities offered to families, contractors, and provider agencies and maintain a record of needs.
  8. Participate in scheduled Service Coordination meetings and training sessions to maintain awareness of current issues which may impact the coordination of cases for individuals in service.
  9. Participate in staff training activities and staff meetings, as outlined in the agency's staff development curriculum.
  10. Work flexible schedules or within various environments as agency and supervisor deems necessary. Regular attendance and promptness to assigned shifts is essential. The responsibilities of this position may require staff to be able to work irregular hours, extended hours, and/or be available for on-call work hours, evenings, nights, weekends, holidays, etc.
  11. Follow agency procedures when purchasing items on behalf of the agency.
  12. Follow agency policies and procedures in carrying out duties.
  13. Advise supervisor and/or the Chief Executive Officer (CEO) of concerns and problems that arise within the agency.
  14. Safeguard all agency funds and property.
  15. Maintain confidentiality per federal, state, and agency policies and procedures.
  16. Performs other duties as assigned by the Service Coordination Director, CEO, and/or designee.
  17. Other reasonable duties as may be assigned.

QUALIFICATION REQUIREMENTS: The requirements listed below are representative of the knowledge, skill, and/or ability necessary for success. An individual must be able to perform each essential duty satisfactorily.

MINIMUM/PREFERRED QUALIFICATIONS:

Minimum: Personnel hired by the community centered board to provide service coordination shall meet at least one (1) of the following standards:1. A bachelor's level degree or, 2. Five years of experience in the field of developmental disabilities; or,3. Some combination of education and experience appropriate to the requirements of the position. Persons hired into this position must be licensed drivers and have reliable transportation. If the current license is from out-of-state, a valid Colorado Driver's License is required within ten days of hire. Background check, drug and alcohol check, reference check, and motor vehicle checks must be passed. Current motor vehicle insurance is required. Driving is an essential job duty.

Preferred: Bachelor's Degree in the field of Human Services (i.e. Psychology, Social Work, Education, Rehabilitation, Community Service, Sociology, Criminal Justice, etc.) and one year of experience in the field of human services working with individuals with developmental disabilities (children and adults).

While performing the duties of this job, the employee will regularly sit, walk, and stand. Occasionally, movement that requires twisting at the neck and/or trunk more than the average person, squatting/ stooping/kneeling, reaching above the head, and forward motion will be required. The employee will continuously be required to repeat the same hand, arm, or finger motion many times. Manual and finger dexterity are essential to this position. Specific vision abilities required by this job include close, distance, depth perception and telling differences among colors. The employee must be able to communicate through speech with clients and the public. Hearing requirements include conversation in both quiet and noisy environments. The employee will occasionally lift and/or push and pull up to 50 pounds. Lifting may require floor to waist, waist to shoulder, or shoulder to overhead movement. This position demands tolerance for various levels of stress.

The noise level in the work environment is usually moderate to loud. Working conditions normally involve inside environments. This position requires working irregular or extended work hours. The employee is directly responsible for the safety and well-being of clients.

LANGUAGE SKILLS: Able to read and comprehend simple instructions, correspondence, and memos. Able to read, analyze, and interpret general business periodicals, professional journals, technical procedures, manuals, or governmental regulations. Able to write reports, business correspondence, and procedure manuals. Able to effectively present information and respond to questions from groups of administrators, managers, employees, clients, clients' parents, and/or the general public is required.

MATHEMATICAL SKILLS: Able to apply mathematical skills to include concepts such as fractions, percentages, ratios, and proportions to practical situations. Able to compute rate, ratio, and percent and to draw and interpret bar graphs. Able to work with mathematical concepts such as probability and statistical trend analysis.

REASONING ABILITY: Able to deal with problems involving a variety of concrete variables in standardized situations. Able to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Able to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Able to define problems, collect data, establish facts, and draw valid conclusions.


  • SUPERVISORY RESPONSIBILITIES:

The Community Relations Director provides supervision of this position. In general, this position requires that the individual work independently approximately 85% of the time.


  • OTHER SKILLS and ABILITIES:

Able to work with others, with system issues, and able to work within designated time frames. Patience in dealing with staff, individuals in service, and parents. Able to develop effective working relationships with staff, individuals in service, and the community. Good communication skills and being able to work with others as a team is a must. Able to communicate clearly and concisely, both orally and in writing. Able to perform duties in accordance with agency policies and procedures.
