Position Summary

In coordination with the leadership team, you will represent the facility. This position requires maintaining a professional demeanor with the public and assisting in the move-in process for our villagers, as well as supporting the Social Service Director.

Move-In Coordinator Responsibilities

  • Demonstrates commitment to the company's mission, values, and standards of ethical behavior.
  • Receives and reviews inquiries regarding the facility via telephone, email, fax, and in person. Extracts information from referrals and completes an information sheet for team review.
  • Assists in making arrangements with discharge planners, families, and physicians to accomplish admission to the facility.
  • Prepares admissions paperwork for signatures before the responsible party's arrival.
  • Assists in the resident admission orientation program in accordance with established policies and procedures, including placement of admission, welcome card, inspection of resident's room, and escorting of residents to their room.
  • Reviews or reads the admission packet to the new admit and / or their responsible party, assists with the completion of the admission packet, and obtains signatures on all necessary forms within 24 hours of admissions. Delays are reported to the Administrator and / or Business Office Manager with the date of projected completion.
  • Obtains all necessary cards to verify the identity and financial status of the resident (i.e., HMO, Medicare, or Medi-Cal cards) and verifies entitlement to HMO, Medicare, etc., coverage. Completes the cost of the patient. Checks Common Working File for Insurance / Payment Accuracy.
  • Provides villagers with admission information packets (e.g., villager rights, notice of privacy practices, admissions contract, etc.). Reviews with the villager, as necessary. Obtains the villager / responsible party's signature on all required permits, releases, authorizations, etc.

Marketing Coordinator

  • Develop and implement a team-based, facility-level Marketing Plan to include analyzing the local healthcare system, identifying and meeting the needs of customers, and determining core competitive advantages.
  • Make external sales calls on the target market to execute the facility Marketing Plan.
  • Develop and maintain social media sites, collaterals, and other print media
  • Develop and maintain a database for outreach and public relations
  • Serve on key community committees or attend relevant community meetings as the representative
  • Obtain knowledge of changing market needs and recommend changes in features, benefits, amenities, or rates
  • Work with the NHA / DON to identify and develop an Admission Support Team.
  • Lead the development of a Public Relations campaign, including media contacts, press releases, event management, etc., that would extol the virtues of the facility to the local community.
  • Develop and implement Marketing Plan tactics to include special events targeted at community education, establishing the facility as the expert in skilled nursing, specialty programs, and rehabilitation care.
  • Deliver a presentation on the non-traditional nursing home Model and the community to local groups.
  • Participating as an active member in key community goals and activities
  • Monitor and evaluate customer satisfaction surveys. Work with the household, clinical, and leadership teams to develop action plans to address satisfaction concerns.
  • Social Service Assistant Responsibilities

  • Assists in interviewing and obtaining information from villagers and families to assist the Social Services Director in the development of psychosocial history, assessment, and care plan upon admission, quarterly, and upon change of condition for social service needs.
  • Assist villagers and families with transition and adjustment to placement in the facility.
  • Assists with the determination of the legal status of villagers upon admission, provides information and obtains copies of advance directives, and assures that resident and family wishes regarding treatment (including Do Not Resuscitate (DNR) and withholding treatment) are implemented in accordance with Company policy.
  • Informs the villager / family of the villager's personal and property rights.
  • Provide input on social, psychological, and emotional needs of villagers / family to supervisor.
  • Ensures villagers have required evaluations for visual, audio, mental health, and dental needs, and follows up as appropriate.
  • Arrange villagers' appointments including transportation.
  • Assists villagers and families in finding and utilizing financial, legal, mental health, and other community resources.
  • Participate in villagers' and family council meetings as requested.
  • May perform documentation duties in accordance with Company charting and documentation policies and procedures and government regulations. Ensures documentation is accurate, clear, informative, and descriptive of villager's condition, care provided, and villagers' response to care.
  • Assists in discharge planning services including arrangement for follow-up services, transfers to other facilities, and post-discharge plan of care, as well as orientation for villagers and family regarding discharge.
