What are the responsibilities and job description for the Community Service Coordinator -Clay position at Inspire to Rise, Inc?
The Community Service Coordinator is responsible for assessing a families’ strengths and needs and makes service referrals as needed. This position enhances family functioning by building rapport and engaging the family all the while teaching problem solving skills, providing information and improving the families’ support system, assesses parenting and communication skills. Services provided shall be comprehensive and well-coordinated. In addition, this position wil be supporting participants in correctional facilities.
Essential Functions:
- Provide case management, concrete supports, and community resource services in the home.
- Develop a service/treatment plan in collaboration with the family to establish family goals and guide service delivery.
- Link families to different service providers to address their educational, medical, speech, growth, dental, and developmental needs.
- Work in collaboration with the Community Education and the Community Behavioral Health Team.
- Document all services and engagements with the families we serve and maintain all information in the case file.
- Perform psychosocial and other functional assessments through the review of records and interview of clients, family and others.
- Participate in monthly treatment team meetings and clinical staffing’s to evaluate the condition of and treatment effectiveness for the participant.
- Monitor the condition of the participant to evaluate success of therapy, track progress, and adapt treatment/ service plans as required.

Work Environment: This job operates in a professional office environment, community locations, jails and in high risk family homes. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
Position Type and Expected Hours of Work: This is a full-time position. This position requires a flexible work schedule.
Travel: This position requires up to 75% travel. Frequently, travel is within the surrounding counties.
Required Education and Experience: Bachelor’s Degree in Human Services
