The Intake Coordinator coordinates activities of order intake, evaluation of admission criteria, referral/authorization of cases, and transition of care for the Home Health Agency; staffs initial nurse, contacts patient/family member, and tracking of referrals. The intake coordinator acts as liaison to case management ensuring patient care.

What Will You Come With
- Minimum of some college coursework
- Minimum of one (1) year of nursing experience, in home healthcare preferred
- Active Practical Nurse (LPN) license in the State of Florida.
- Knowledge of the Home Health Services
- Knowledge of referral/authorization process
- Knowledge of computer clinical programs such EHR/EMR system(s)
- Great communication skills, both written and spoken.
- Bi-lingual preferred
- Strong organization skills and be detail oriented
- Ability to maintain confidentiality of sensitive information.
