Integrated Life Choices is seeking a Case Manager for individuals with developmental and intellectual disabilities in our services. We are looking for a dedicated individual to join our team in Lincoln, NE.
Position Summary: Oversees the integrity of all medical related aspects of the assigned ILC caseload of individuals supported including medications, appointments, pharmacy relations, and quality reviews for all individuals ILC supports by performing the following duties. Admission data entry and assist the Registered Nurse in health care plans for our Colorado Services.

Wage: $18-$20 an hour.

Duties and Responsibilities include:
- Tracks and process medical appointments, annuals, and consultation forms.
- Enters and adjust medications for established individuals that ILC supports.
- Maintain appointments, inputs caseloads, responds to scomms, and other maintenance as needed in Therap.
- Acts as a support for non-nursing medical questions/concerns caregivers and fellow ILC office staff may have, and directs questions to nursing when appropriate.
- Acts as liaison between Physician offices, pharmacies, and nurses.
- Maintains all confidential records.
- Routes all Physician contact forms for individuals ILC supports per ILC policies.
- Routes all GERs for individuals that ILC supports.
- Serves on Quality Improvement and Therap Committees.
- Answer phones and direct calls accordingly.
- Conducts medication and confidential book reviews for all individuals ILC supports.
- Track annual appointments, and send out appropriate documents to HHP / PCA / FCG’s.

Requirements
- Minimum of 19 years of age
- Associate degree (A. A. / A. S.) or equivalent from two-Year College or technical school; and two to four years related experience and/or training; or equivalent combination of education and experience is preferred.
- Successfully complete and pass a Background Check.
Certificates and Licenses:
- Valid Driver's License (Required)
- Experience as a CNA, or MA (Preferred)
