What You'll Do!
Our Case Managers (known as "Professional Partner") serve a vital role to support consumers in our Family & Youth Investment program .
This position provides wraparound services to individuals under the age of 26, who are diagnosable with a mental illness (Serious Emotional Disturbances or SED) and residing in Region 5 Systems' geographical area (southeast Nebraska). Persons served are at risk of being placed outside of the home, school, or community and are at risk for involvement in the juvenile justice and / or child welfare systems, which may include committing a criminal offense or behavioral health problems at school or home. The population served is primarily school-aged youth but also includes those individuals who are transitioning from the youth system to the adult system and who are at risk of entering, or have already entered, the adult emergency system.
You will use your problem-solving skills to work with your program participants (15 caseload maximum!) to develop a unique and creative plan to build on the natural support systems in their lives. In other words - you will help set the stage for success.
