Our organization is currently looking for dynamic, creative Individuals to partner with youth/family to provide High Fidelity Wraparound Facilitation. The Wraparound Facilitator will deliver services guided by Wraparound Principles: Family voice and choice, Team-based, Natural Supports, Collaboration, Community-based, Culturally Competent, Individualized, Strength-based, Unconditional care and Outcome-based.
This position will work Monday - Friday, 8am - 5pm, but have the opportunity for flexible scheduling. The position will work with youth/families in Marion, Hancock, Hamilton, Hendricks counties.
Responsibilities:
- Guide the family engagement process by exploring and assessing strengths and needs through the documentation of the family story
- Comprehensively assess the youth, including administering the CANS assessment tool
- Guide the Plan of Care process by communicating the Family’s vision and ensuring that family’s voice, preferences, and vision are central to planning and delivery of services.
- Coordinate with team members to ensure POC is developed, written, and approved by the DMHA.
- Monitor progress toward meeting established goals, and adherence to action plans.
- Assists in the development of natural supports and non-traditional community resources needed for youth and their families to reach their vision.
- Facilitate, coordinate, and attend monthly team meetings
- Offer consultation and education to all team members regarding the values and principles of the wraparound model.
- Ensure all assessments and service-related documentation is gathered and reported as required.
- Guide the Transition of the youth and family from wraparound services to State Plan or other community-based services, when indicated.
Requirements:
- Experience with Systems of Care preferred
- Available to work weekdays and evenings; Typical hours are Monday through Friday 9 am - 5 pm
- Ability to work independently in a home office that is private and free from disruptions.
- Ability to maintain professional relationships with a variety of individuals
- Comfortable working in a range of settings, including: homes, the community, and schools
- Strong communication skills.
- Strong organizational skills
- Must hold a valid driver's license with acceptable driving record and provide own, insured, transportation
- Bachelor's degree in Social Work, Psychology, Human Services, or other related field
- Two years experience with children ages 6-17 that have been identified as Seriously Emotionally Disturbed (SED).
- Previous experience with Wraparound Facilitation is strongly desired.
