Title:

Role:
Navigates patients through non-medical and medical issues
Qualifications:
Individual must meet at least one of the following:
- A Registered Nurse (RN);
- A Licensed Practical Nurse (LPN) with two years of lived experience;
- An individual with a baccalaureate (BA) degree and two (2) years of lived experience;
- An individual with an associate’s degree and four (4) years of lived experience; or
- A Certified Medical Assistant with four (4) years of lived experience
Qualities:
- Harm-reduction approach to assisting patients with Substance Use Disorder (SUD)
- Ability to use computer systems, computer literacy, and knowledge of word processing
- Interest/proficiency in working with marginalized populations
- Ability to interact with patients in a culturally relatable manner
- Respect for patient confidentiality and privacy
- Ability to communicate clearly verbally and in writing
Responsibilities:
- Become knowledgeable with the continuum of care, as well as all substance use treatment services, providers, and advocacy agencies within the defined service area
- Assist with identification of patients with a substance use or co-occurring mental health disorders and to receive referrals of eligible patients
- Maintain up-to-date information about the effects of various substances, withdrawal symptoms, and treatment options in order to effectively educate and counsel patients
- Promote harm-reduction strategies based on clients’ goals/preferences and circumstances
- Serve as a liaison and collaborator between patients, healthcare providers, pharmacies, insurance companies, and other professionals
- Help patients navigate the healthcare system and schedule appointments
- Understanding of HIPAA regulations to maintain patient privacy and confidentiality
- Use motivational interviewing techniques to communicate with patients in a respectful, culturally appropriate, non-judgmental manner
- Participate in front office duties including but not limited to: scheduling patients, verifying insurances, creating patient charts, and answering phones.
- Knowledge of medical records management and documentation
- Navigators should also be available for any activities that support their role
