Shared Living Provider
We are looking for a family like environment in a two adult house hold with no children, Perferrably a home with experienced in behaviors.
Integrity Alliance, LLC. is seeking Shared Living Providers (SLP’s) to provide residential services in the SLP’s home for individuals with developmental disabilities.
It is vital that SLP’s maintain a safe and healthy home environment for the person supported. SLP’s will have basic home support and related responsibilities which include but are limited to providing/supporting nutritional meals, providing transportation to appointments, providing transportation to and from day services and activities, providing and supporting access to community activities, and providing supervision as outlined in the persons plan (ISP).
The SLP is responsible for teaching and supporting the person with their life choices.
Integrity Alliance works to match those with developmental disabilities with trained Shared Living Providers that share the same interests and hobbies. The location, size, structure, and family setup of the home are all taken into consideration when matching an SLP with a person needing supports.
We are looking for a year long minimum commitment but long term preferred.
Requirements:
- Valid Driver’s License
- Valid Car Insurance
- Homeowners/Renters Insurance
- Ability to pass all required background checks.
- Completion of all required training.
- Must understand and comply with agency and state policies and procedures.
- Must attend and participate in meetings.
- CNA or Nursing License preferred
- Must maintain clean and orderly home.
- Must be at least 19 years of age.
- Must be able to life 50 pounds, kneel, twist, reach, crawl, run, etc.
