Job Description:
As the Director of Case Management, you will be responsible for directing and coordinating all aspects of case management, including utilization management, discharge planning, and cost/quality management programs. You will work closely with clinical staff, physicians, and other healthcare professionals to ensure the efficient and effective delivery of services to patients. You will also lead efforts to improve patient outcomes, streamline care processes, and manage resources effectively.
This role requires strong leadership, a deep understanding of healthcare systems, and experience managing case management programs. You will be instrumental in improving operational efficiency and ensuring compliance with healthcare regulations.
Responsibilities:
- Lead and oversee all case management activities, including utilization management, discharge planning, and cost/quality management.
- Coordinate with healthcare teams to ensure effective patient care delivery and optimal patient outcomes.
- Monitor and assess the performance of the case management team, providing guidance and support as needed.
- Develop and implement policies and procedures for case management operations.
- Ensure compliance with state and federal regulations and industry best practices.

Requirements:
- Education: Graduate of a program of Registered Nursing (RN).
- Experience: Minimum of two years of Case Management experience in utilization management, case management, discharge planning, or other cost/quality management programs.
- Licensure: Current RN license in the state of Iowa or a multistate license allowing you to work in Iowa.
- Skills: Strong leadership and communication skills, ability to manage and coordinate multiple tasks, and a thorough understanding of case management principles and healthcare regulations.
