Position Summary
The Weekend Admission Coordinator is a valuable member of the Hospice team and will be responsible for meeting the
needs of our patients outside normal business hours.
Essential Duties and Responsibilities
The following statements describe the principal activities of this position identified and shall not be construed
as a detailed exposition of all the duties that may be inherent in this position.
• Documents each call, intervention, and appropriate followup in an On-Call Coordination note;
• On-Call Coordination notes to be used to provide report log for the office staff and Hospice team;
• Notifies RN on-call when admission visit is required;
• Schedules admission visits in electronic medical record (EMR);
• Updates schedules for field staff when needed;
• Contacts physician to obtain admission orders or comfort orders, as needed;
• Receives orders and updates EMR with medications/treatment changes when needed;
• Collaborates with Care Support for referrals received outside normal business hours;
• Performs follow up calls to patients who have discharged home from the hospital and are awaiting Hospice admission;
• Enters new referrals in EMR;
• Coordinates new referrals with Hospice On-Call team;
• Uploads new patients into Optum and orders DME;
• Processes workflows after hours to ensure timely care;
• Represents the Company and the Hospice department in a courteous and professional manner at all times;
• Complies with Compliance Plan;
• Complies with HIPAA Policies. Maintains confidentiality of information relating to the patient and family. Will discuss only those aspects necessary to the care and treatment of patient and family with those directly involved in the patient’s care;
• As business needs may change, may perform additional responsibilities assigned by supervisor that are reasonably related to the position.
Education and Experience Requirements
Education:
• Graduate of an approved school of Registered Professional Nursing or Licensed Practical/Vocational
Nursing;
• Current LPN licensure in the State of Missouri;
• Possess and maintain current CPR certification and TB test.
Experience:
Must have one of the following:
• Minimum one (1) to two (2) years’ recent acute care experience in an institutional setting;
• Minimum one (1) to two (2) years’ recent experience in home care preferred;
• Hospice experience preferred but not required.
