Job Responsibilities
- Provide high-quality patient care to residents in post-acute facilities
- Be accountable for following and adhering to resident care plans
- Measure and monitor vital signs, reporting to your supervisor as necessary
- Observe the conditions of residents during your shift, reporting any concerns to the broader nursing team
- Serve meals to residents and assist with feeding as needed
- Support residents with activities of daily living (ADLs) such as personal hygiene, mobility, and toileting
- Clean and sanitize surfaces, keep residents' rooms clean, change linens, and ensure a hygienic environment
- Serve as a liaison between medical staff and residents, and communicate with visitors
- Move residents in and out of wheelchairs and beds, reposition them as needed
- Offer a compassionate ear to residents and provide companionship
