Role Overview
We are seeking a compassionate and skilled Registered Nurse to join our team as a Home Hospice Nurse. As a key member of our interdisciplinary team, you will provide end-of-life care to patients in the comfort of their own homes.

Responsibilities
- Provide home-based care to hospice patients with a prognosis of six months or less to live.
- Collaborate with an interdisciplinary team to develop and execute individualized care plans.
- Conduct thorough patient assessments, monitor vitals, evaluate breathing, and manage pain using prescribed medications and treatments.
- Ensure the availability of necessary medical equipment, oxygen, and medications.
- Educate patients and families about care plans, medications, hospice goals, and what to expect.
- Offer holistic care that addresses the physical, emotional, and spiritual needs of patients and their families.
