Job Description
Interim HealthCare of West Michigan is a leading provider of post-acute care services in the region. We are currently seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Hospice division.
The ideal candidate will have experience in home health and hospice care, with strong assessment and documentation skills. They will be responsible for completing ongoing assessments of patients' physical status and psycho-social status, developing plans of care in collaboration with the Interdisciplinary Group (IDG), and ensuring that equipment and supplies are available.
