We are seeking a skilled and compassionate Physical Therapist Assistant to join our Interim HealthCare Home Health and Hospice team.
Job Summary
This part-time or full-time opportunity allows you to work closely with patients in their own homes, providing one-on-one care and support.
Key Responsibilities
- Assist treating physical therapists in developing and implementing patient plans of care.
- Provide services as ordered by physicians and document patient progress accurately.
- Counsel and educate patients and caregivers on healthcare-related needs.
- Communicate effectively with interdisciplinary teams to promote coordination of patient care.
Requirements:
- Current licensure as a physical therapist assistant in the state of Michigan.
- At least two years of experience as a physical therapist assistant within the last five years.
- Active CPR certification.
- Previous Home Health experience is highly valued.
