Job Description
As a Home Health Registered Nurse, you will play a vital role in delivering comprehensive care to patients with illnesses, injuries, and chronic diseases. Your responsibilities will include:
- Providing home-based care to patients and collaborating with a team of physicians and other healthcare professionals
- Examine patients, conduct assessments, document progress, and report changes to their physician
- Educating patients on their plan of care, prescribed medication, therapy, diet, and exercise
- Coaching family members on patient safety and home care management
- Ensuring goals are met and coordinating discharge from services
Required Skills and Qualifications
To be successful in this role, you will need:
- A registered nurse license in the state of Michigan
- At least one year of clinical experience as a registered nurse
- Current CPR certification
- Reliable transportation with a valid driver's license and current car insurance
