About Our Home Health LPN Position

Your Responsibilities
- Provide high-quality, patient-centered care to individuals in their homes, under the supervision of a Registered Nurse (RN)
- Collaborate with a multidisciplinary team to deliver comprehensive care and support to patients and their families
- Conduct thorough assessments, develop personalized plans of care, and document progress and changes as needed
- Work closely with RNs to implement patient care plans and educate patients on self-care techniques
- Educate family members on patient care, medication management, and home safety
- Collaborate with the interdisciplinary team to coordinate patient care and plan for discharge

Requirements and Qualifications
- Licensed Practical Nurse (LPN) with an active, unrestricted license in the State of Michigan
- At least one year of clinical experience as a Licensed Practical Nurse (LPN)
- Current CPR certification
- Reliable transportation and a valid state driver's license with current car insurance
Salary : $55,000 - $65,000
