Job Requirements:
We are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice Registered Nurse. As a member of our interdisciplinary team, you will provide end-of-life care to patients and their families in the comfort of their own homes.
About the Position:
This role requires a skilled and empathetic nurse who can provide holistic care that addresses the physical, emotional, and spiritual needs of patients and their families. You will work closely with an interdisciplinary team to develop and execute individualized care plans, ensuring the best possible quality of life for our patients.
Key Responsibilities:
- Provide comprehensive patient assessments and develop care plans that meet the unique needs of each patient
- Manage pain and symptoms using prescribed medications and treatments
- Collaborate with the interdisciplinary team to ensure seamless care coordination
- Educate patients and families about care plans, medications, and hospice goals
Qualifications:
- Active, unrestricted RN license in the State of Michigan
- Minimum of one (1) year of nursing experience (hospice or home health experience preferred)
- Experience in community health or Medical-Surgical settings (a plus)
- Reliable transportation, a valid driver's license, and current car insurance
