Job Description and Responsibilities
As a Hospice Intake Nurse, you will serve as a contact telephone liaison person interfacing with hospice referral sources, outside agencies, health professionals, and field staff. You will receive and coordinate all incoming calls, providing hospice patients/clients and referral sources with basic information to assist in accessing appropriate services.
You will communicate with the hospice patient/client and family regarding arrangements for the initiation of care/services. You will complete intake screening, including obtaining, documenting, and analyzing all required information to make a preliminary admission decision.
