  • Competitive salary range : $55k - $65k annually

Responsibilities :

  • Provide home-based care to patients under RN supervision
  • Work with a team to deliver quality patient care
  • Take vitals, do assessments, and document progress
  • Assist in carrying out patient's plan of care and educate family members
  • Collaborate with interdisciplinary team to coordinate patient care

    • Requirements :

  • Active LPN license in Michigan
  • At least one year of clinical experience as an LPN
  • Current CPR Certification
  • Reliable transportation with valid driver's license and current car insurance
