About the Role
We are seeking a skilled Speech Language Pathologist (SLP) to join our team in providing high-quality, home-based speech therapy services to patients with communication, swallowing or cognitive difficulties due to an illness or injury.
You will work as part of a multidisciplinary home health team focused on the patient's plan of care and established goals. Your responsibilities will include observing and evaluating patient abilities, conducting assessments, documenting progress, and ensuring patient goals are met.
In this role, you will also educate patients and families on their plan of care, therapy goals, safety practices, and self-care. Previous experience in home health is preferred but not required.
