Intake LPN or RN Responsibilities
- Serve as a liaison between hospice referral sources, outside agencies, healthcare professionals, and field staff
- Receive and coordinate incoming calls, providing basic information to assist in accessing services
- Communicate with patients and families regarding arrangements for care and services
- Complete intake screenings, documenting and analyzing required information for admission decisions
- Log referrals, prepare reports, and respond to inquiries
- Provide telephonic contact and on-call support as needed
- Actively promote care and services to prospective patients
- Where permitted by law, receive verbal orders from physicians to initiate care and services
- Collect and enter customer information into our management information system
Intake Nurse Essential Requirements
- Graduate of an accredited school of nursing
- Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in Michigan
- One year of experience as a LPN or RN
- Prior hospice or home health care experience preferred
Salary : $50,000 - $60,000
