- Key Responsibilities:
- Conduct thorough assessments of patients' physical and emotional needs
- Develop and implement personalized care plans in collaboration with the Interdisciplinary Group (IDG)

- Ensure timely and accurate documentation of patient services
- Attend regular IDG meetings to discuss patient progress and adjust care plans as needed
Requirements
To succeed in this role, you must have:
- An active, unrestricted RN or LPN license in Michigan
- At least one year of relevant experience in home health or hospice care
- Strong communication and interpersonal skills
- Able to work independently and collaboratively as part of a team
- Familiarity with electronic medical records (EMRs) and other software applications
