Your Responsibilities
- Conduct ongoing assessments of patients' physical status and psycho-social status.
- Develop and implement care plans in collaboration with our interdisciplinary group, patients, and physicians.
- Provide education and interventions to support effective pain management and symptom relief.
- Maintain accurate and timely documentation of patient services provided.
Requirements
- RN/LPN with active, unrestricted license in Michigan.
- At least one year of relevant experience as an RN or LPN.
- Prior home health experience is a plus.
- Current CPR certification required.
