Job Responsibilities:
- Complete ongoing assessments of patients' physical status and psycho-social needs
- Initiate and develop plans of care in collaboration with interdisciplinary teams and healthcare professionals
- Provide education and interventions to support effective pain management and symptom control
- Ensure timely documentation of patient services and communicate effectively with healthcare teams
Requirements:
- RN/LPN licensure in the State of Michigan
- At least one year of relevant experience as an RN or LPN
- Prior home health or hospice experience preferred
- Current CPR certification
