RN/LPN Responsibilities
- Complete ongoing assessments of the patient's physical status and psycho-social status
- Initiate and develop a plan of care in collaboration with the Interdisciplinary Group (IDG), patient/family, and attending physician
- Provides education and interventions to support effective pain and symptom management
- Ensures equipment, supplies, and necessary items are available
- Make revisions to the IDG plan as the patient's/family's status and needs change
- Accurately document patient/family services provided
- Regularly attend IDG meetings
RN/LPN Requirements
- Active, unrestricted RN/LPN license in the State of Michigan
- At least one year of relevant experience as an RN or LPN (required)
- One year of management experience (preferred)
- One year of home health experience (preferred)
- Current CPR Certification
- Community health and Medical-Surgical experience (preferred)
- Reliable transportation with valid driver's license and current car insurance
