RN/LPN Responsibilities
- Conduct ongoing assessments of patients' physical status and psycho-social needs, working collaboratively with the Interdisciplinary Group (IDG) and attending physicians.
- Develop and implement individualized plans of care, addressing patients' pain management and other symptoms.
- Ensure equipment, supplies, and necessary items are readily available to support patient care.
- Maintain accurate and timely documentation of patient services provided, regularly attending IDG meetings as needed.
RN/LPN Requirements
- You must hold an active, unrestricted license in the State of Michigan as an RN or LPN.
- A minimum of one year of relevant experience as an RN or LPN is required, with preferred qualifications including home health experience and community health background.
- Current CPR certification and reliable transportation with a valid driver's license and car insurance are also required.
