Job Description:
- Provide home-based care to patients with illnesses, injuries, and chronic diseases, often after hospital discharge, under the supervision of an RN
- Work collaboratively with physicians, RNs, CNAs, and aides to deliver quality patient care
- Take patient vitals, conduct assessments, document progress, and report changes to the RN supervisor
- Assist the RN in carrying out the patient's care plan and teaching self-care techniques
- Educate family members on the patient's care plan, medication, and home safety
