Responsibilities:
- Coordinates patient care services, ensuring seamless communication between patients, families, and healthcare professionals.
- Acts as a contact telephone liaison person interfacing with hospice referral sources, outside agencies, health professionals, and field staff.
- Receives and coordinates all incoming calls, providing hospice patients/clients, and referral sources with basic information to assist in accessing appropriate services.
- Communicates with the hospice patient/client and family regarding arrangements for the initiation of care/services.
Requirements:
- Graduate of an accredited school of nursing.
- Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of Michigan.
- One year of experience as a Licensed Practical Nurse (LPN) or Registered Nurse (RN).
- Prior hospice or home health care experience preferred.
