Nurse Responsibilities:
- Provide home-based care to patients with illnesses, injuries, and chronic diseases under RN supervision
- Collaborate with interdisciplinary teams to deliver quality patient care
- Take patient vitals, conduct assessments, document progress, and report changes to RN supervisor
- Assist RN in carrying out patient's plan of care and teaching patient self-care techniques
- Educate family on patient's plan of care, medication, and home safety
- Coordinate patient care and plan for discharge

Requirements for Nurses:
- Licensed Practical Nurse (LPN) with active, unrestricted license in Michigan
- At least one year of clinical experience as an LPN
- Current CPR Certification
- Reliable transportation with valid driver's license and current car insurance
Salary : $55,000 - $65,000
