Responsibilities:
- Provide personalized care to patients in their homes, often after hospital discharge or rehabilitation
- Collaborate with physicians and oversee LPNs, CNAs, and aides providing patient care
- Conduct thorough assessments, document progress, and report changes to physicians
- Educate patients on their treatment plan, medication, therapy, diet, and exercise
- Coach family members on patient care, medication, and home safety
- Ensure patient goals are met and coordinate discharge from services

Requirements:
- Active, unrestricted Registered Nurse (RN) license in Michigan (BSN not required)
- At least one year of clinical experience as a Registered Nurse
- Current CPR Certification
- Reliable transportation with a valid state driver's license and current car insurance
