Job Requirements

To succeed in this role, you will need:

• Current licensure as a physical therapist assistant in the state of Michigan
• Two years of experience as a physical therapist assistant within the last five years
• Active CPR certification
• Previous home health experience is a plus

Key Responsibilities:

1. Contribute to ongoing interdisciplinary assessments of patients
2. Collaborate with the treating physical therapist and follow the plan of care
3. Develop and evaluate the plan of care with the patient, representative (if any), and caregiver(s)
4. Deliver services ordered by physicians as indicated in the plan of care
5. Document patient progress and response to treatment with the treating physical therapist
6. Submit timely and accurate documentation addressing treatment and interventions provided
7. Counsel and educate patients and their families to promote progress toward mutually established goals
8. Communicate effectively with other healthcare professionals to coordinate patient care and plan for discharge
9. Access personal health information necessary to fulfill duties and responsibilities
