Job Requirements
To succeed in this role, you will need:
- Current licensure as a physical therapist assistant in the state of Michigan
- Two years of experience as a physical therapist assistant within the last five years
- Active CPR certification
- Previous home health experience is a plus
Key Responsibilities:
- Contribute to ongoing interdisciplinary assessments of patients
- Collaborate with the treating physical therapist and follow the plan of care
- Develop and evaluate the plan of care with the patient, representative (if any), and caregiver(s)
- Deliver services ordered by physicians as indicated in the plan of care
- Document patient progress and response to treatment with the treating physical therapist
- Submit timely and accurate documentation addressing treatment and interventions provided
- Counsel and educate patients and their families to promote progress toward mutually established goals
- Communicate effectively with other healthcare professionals to coordinate patient care and plan for discharge
- Access personal health information necessary to fulfill duties and responsibilities
