Job Summary
We are seeking a skilled Home Health Speech Language Pathologist to join our team. As a Home Health SLP, you will work with patients in their homes to help them overcome communication, swallowing, and cognitive challenges.
About the Role
You will be responsible for providing home-based speech therapy to patients, working as part of a multidisciplinary team to achieve patient goals, conducting patient assessments, documenting progress, and educating patients and families on care plans.
Key Responsibilities
- Provide home-based speech therapy to patients with communication, swallowing, or cognitive difficulties
- Work collaboratively with a home health team to achieve patient goals
- Conduct patient assessments and document progress
- Educate patients and families on care plans and self-care practices
Requirements
- Graduate of an accredited Speech Language Program
- Active SLP license in Michigan
- 1 year of experience as an SLP within the last 5 years
- Good clinical judgment, strong interpersonal skills, and resourcefulness
