Key Responsibilities
- Provide home-based speech therapy to patients with communication, swallowing, or cognitive difficulties due to illness or injury.
- Work collaboratively as part of a home health team to achieve patient goals and deliver comprehensive care.
- Conduct patient assessments, document progress, and ensure patients meet established goals.
- Develop and implement individualized treatment plans to address patients' unique needs.
- Educate patients and their families on therapy goals, safety practices, and self-care techniques.
Requirements
- Graduate of an accredited Speech Language Program and active SLP license in Michigan.
- Minimum one year of experience as an SLP within the last five years.
- Excellent clinical judgment, strong interpersonal skills, and a compassionate approach.
- Previous Home Health experience is preferred but not required.
