**Job Title:** LVN (Licensed Practical Nurse)
We are seeking an experienced Licensed Practical Nurse (LPN) to provide high-quality patient care in the East Houston area.
This role requires strong patient assessment skills, excellent communication skills, and the ability to work independently with minimal supervision. Reliable transportation is also a must.
About the Role:
- Develop and implement individualized care plans in accordance with patient needs and healthcare provider orders
- Perform various medical tasks, including medication administration, wound care, treatments, and procedures as assigned
- Monitor patients' conditions, reporting any changes or concerns to our RN team members
- Accurately document all observations, interventions, and evaluations related to client care management and services provided
Requirements:
- Active LPN license in the State of Texas
- Current CPR Certification
- Updated PPD Test
- Minimum one year of experience in healthcare, preferably in home care, home health, hospital, or long-term care environments
- Able to pass a drug test and criminal background check at the time of interview
