PRN Hospice Bereavement Coordinator

We are seeking a compassionate and experienced Hospice Bereavement Coordinator to join our dedicated team. In this role, you will provide essential emotional support and guidance to patients' families and caregivers during one of the most challenging times of their lives. As a key member of the hospice care team, you will develop and implement grief support programs, facilitate support groups, and ensure that families receive ongoing bereavement care after the loss of their loved one. As a Hospice Bereavement Coordinator, here’s a big-picture view of what you’ll do:
- Primarily responsible for coordinating the assessment of grief following the death of the patient, determining the bereaved’s needs and ensure timely and appropriate services in response.
- May provide bereavement individual or group counseling in response to anticipatory grief or bereavement as qualified, or makes appropriate referrals for such services.
- Responsible for collaboration with the Interdisciplinary Group (IDG) to develop an appropriate bereavement plan of care.
- Responsible for coordinating the assessment of grief following the death of the patient, determining the bereaved’s needs and ensure timely and appropriate services in response.
- Acts as a primary resource regarding bereavement information to the community served.
- Participates in the Quality Assessment and Performance Improvement (QAPI) program, including the regular evaluation of the bereavement program.
- Participates in program budget preparation as assigned
