RN Case Manager
Job Location: Southern, Maine
Compensation: $39-$41 /HR
Description of the role:
The RN Case Manager will be responsible for coordinating, managing, and delivering quality patient care within the home health care setting. They will work closely with patients, families, and healthcare professionals to ensure appropriate care plans and interventions are implemented.
Responsibilities:
- Assessing patient needs and developing individualized care plans
- Coordinating and facilitating healthcare services

Requirements:
- Valid RN license in the state of Maine
