At Interim HealthCare of Louisiana, we are seeking a compassionate and skilled Hospice Licensed Practical Nurse (LPN) to join our dedicated team.
About the Role
This is an exciting opportunity for a motivated LPN to work in a dynamic hospice care environment where patient-centered care is our top priority.
- We deliver personalized care and support to patients and their families in their homes or facilities, working under the supervision of a Registered Nurse (RN).
This role requires strong clinical skills, effective communication, and compassion. You will take patient vitals, conduct assessments, document progress, and report changes to the RN supervisor.
In this position, you will be part of an interdisciplinary team that includes physicians, RNs, CNAs, and aides, working together to deliver quality patient care.
Our ideal candidate has experience in hospice care, a graduate degree from an accredited practical nursing program, and active LPN licensure in Louisiana.
Responsibilities
- Taking patient vitals, conducting assessments, documenting progress, and reporting changes to the RN supervisor.
- Assisting the RN in carrying out the patient's plan of care.
- Educating family members on the patient's plan of care.
- Collaborating with the interdisciplinary team to coordinate patient care.
Requirements
To succeed in this role, you must meet the following requirements:
- Graduate of an accredited practical nursing program and active LPN license in Louisiana.
- Hospice LPN experience required.
- CPR certification, valid driver's license, auto insurance, and reliable vehicle.
- Knowledge of state and federal home health regulations.
- Clinical proficiency in nursing, strong communication skills, and compassion.
