Job Description:
As a Hospice Licensed Practical Nurse, your primary responsibility will be to provide quality patient care and work collaboratively with our interdisciplinary team. This includes taking patient vitals, performing assessments, documenting progress, and reporting changes to the RN supervisor. You will also assist the RN in carrying out the patient's plan of care and educate family members on the patient's plan of care.
Required Skills:
To succeed in this role, you will need:
- A graduate degree from an accredited practical nursing program and an active LPN license in Louisiana
- Hospice LPN experience required
- CPR certification, valid Driver's License, auto insurance, and a reliable vehicle
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing, strong communication skills, and compassion
