Job Description:
As a Physical Therapist with Interim Healthcare of the Twin Cities, you will have the opportunity to work independently in the field, visiting patients one on one in their home environment. Your primary responsibility will be to provide physical therapy services as ordered by the physician, developing and implementing individualized plans of care tailored to each patient's needs.
- Assist the Physician or other licensed practitioner in evaluating patient function and performance, selecting interventions, and determining service intensity, frequency, and duration.
- Complete initial and ongoing comprehensive assessments at assigned time points.
- Participate in interdisciplinary assessment of the patient, contributing to the development and evaluation of the plan of care.
- Provide services that are ordered by the physician as indicated in the plan of care.
- Provide interventions and therapeutic treatment in accordance with the physician-certified plan of care and physical therapy professional standards of practice.
