As a Home Health LPN, your primary responsibilities will include:

• Providing home-based care to patients with various medical conditions under the supervision of an RN.
• Collaborating with a multidisciplinary team, including physicians, RNs, CNAs, and aides, to deliver high-quality patient care.
• Taking patient vitals, conducting assessments, documenting progress, and reporting changes to the RN supervisor.
• Assisting the RN in implementing the patient's plan of care and teaching self-care techniques.
• Educating family members on the patient's plan of care, medication management, and home safety.

To succeed as a Home Health LPN, you should possess:

• A graduate degree from an accredited practical nursing program and an active LPN license in Kansas.
• CPR certification and knowledge of state and federal home health regulations.
• Clinical proficiency in nursing, strong communication skills, and compassion.
