About the Job

As a Home Health Licensed Practical Nurse (LPN) with Interim HealthCare, you will provide home-based care to patients with illnesses, injuries, or chronic diseases under the supervision of an RN.

You will work with a team of physicians, RNs, CNAs, and aides to deliver quality patient care. Your responsibilities will include taking patient vitals, doing assessments, documenting progress, and reporting changes to the RN supervisor.

Collaboration with the interdisciplinary team is essential to coordinate patient care and plan for discharge.
