Responsibilities:
- Deliver high-quality patient care in a home setting under the guidance of an RN
- Collaborate with a multidisciplinary team to develop and implement patient care plans
- Take patient vitals, conduct assessments, document progress, and communicate changes to the RN supervisor
- Teach self-care techniques to patients and educate families on patient care and home safety
- Contribute to the coordination of patient care and planning for discharge
Eligibility Criteria:
- Active LPN license in FL
- Minimum 1 year home healthcare experience required
- CPR certification/BLS certification
- Strong clinical skills, effective communication, and compassion
