Responsibilities:

• Deliver high-quality patient care in a home setting under the guidance of an RN
• Collaborate with a multidisciplinary team to develop and implement patient care plans
• Take patient vitals, conduct assessments, document progress, and communicate changes to the RN supervisor
• Teach self-care techniques to patients and educate families on patient care and home safety
• Contribute to the coordination of patient care and planning for discharge

Eligibility Criteria:

• Active LPN license in FL
• Minimum 1 year home healthcare experience required
• CPR certification/BLS certification
• Strong clinical skills, effective communication, and compassion
