As a key member of our interdisciplinary team, you will provide hands-on personal care, report changes in client condition, perform simple procedures as an extension of therapy or nursing services, and provide incidental/instrumental activities of daily living.

**Job Duties:**
- Assist with activities of daily living such as bathing, dressing, grooming, light housekeeping, meal preparation, shopping, transportation and companionship.
- Help with ambulation, transferring and range of motion exercises.
- Provide medication reminders, document their condition and notify a supervisor of any concerns.
- Evaluate and maintain a safe home environment with unobstructed pathways.
- Engage clients in activities that bring them joy, such as puzzles, games, reading and hobbies.
